Homelessness is the largest social and public health crisis in
California. Illumination Foundation (IF) is a growing non-profit
organization dedicated towards disrupting the cycle of homelessness
by providing targeted, interdisciplinary services in our
recuperative care centers, emergency shelters, housing services and
children's and family programs. IF currently has 13+ facilities
with 22+ micro-communities scattered across Orange County, Los
Angeles County and the Inland Empire.
Job Description:
The VP will provide leadership and be responsible for the oversight
of all healthcare programs throughout Illumination Foundation,
including Recuperative Care, Enhanced Care Management , In Lieu of
Services and other CalAIM programs. Under the supervision of the
Co-CEO will spearhead system-wide initiatives to achieve
organizational goals and improve the skills of our medical
professionals in the care delivery process. The pay range for this
position is $110,000 to 160,000 annually depending on
experience.
Tasks/Responsibilities:
Serves as a key member of the Senior Management team by developing
and implementing new policies and programs, guiding plans for
programmatic expansion through community and hospital outreach, and
monitoring operational compliance concerning licensure and
accreditation to the standards of State and Federal
improvement/assurance requirements.
Oversees and provides direct supervision to all health care service
teams, care managers, children/family programs and special
projects.
Works with clients, providers and multidisciplinary team members to
assess, facilitate, plan and coordinate an integrated delivery of
care across the continuum, including behavioral health and long
term care, for members with hi-risk needs.
Collaborates with the Healthcare Services management team to
achieve successful implementation of IF clinical strategy and
direction.
Develops and implements effective and efficient standards,
policies, protocols, processes and workflows. Utilizes decision
support systems for operational reporting and benchmarks to support
ongoing performance improvement of clinical operations to promote
quality and cost-effective health care for insured and non-insured
members.
Continually refines operational processes and champions review of
team processes, workflows, and activities
Mentors, guides, and develops skills of management team
members.
Develops and oversees Healthcare Services department budget to
ensure budget targets are met.
Manages implementation of analytical studies that quantify the
benefits of Healthcare Services programs to ensure that resources
are appropriately allocated, operational controls exist, and
efficiencies are maximized.
Facilitates care coordination, behavioral health services, and
chemical dependency programs to support the delivery of an
integrated network.
Accountable for ensuring compliance with contractual,
accreditation, regulatory requirements and quality assurance for
all Healthcare Services teams.
Participates or assigns appropriate staff to IF Committees to
represent the Healthcare Services Department.
Ensures effective inter-departmental collaboration and interaction
between Healthcare Services staff and other departments.
Ensures performance reviews of caregivers are conducted with
appropriate coaching and improvement action plans.
Responsible for oversight of educational programs and clinical
training activities and outcomes.
Develops policies and procedures that meet applicable CalAIM
program requirements.
Stays current on regulations and policies impacting the CalAIM
program, health plan operations, and compliance programs and shares
relevant knowledge across the organization.
Communicates confidently and persuasively to all audiences,
including external stakeholders.
Builds, mentors, and manages teams of Associates, Managers, and
other Directors to sustain and grow IF's community services.
Collaboratively leads initiatives to ensure staff and client
safety, quality, infection control, risk management and
satisfaction of services. Oversees comprehensive case management,
discharge planning, community service referrals, comprehensive
health care, and housing placement.
Assesses resources and activities needed to achieve organizational
goals and to make recommendations for government and private grant
funding.
Consults on clinically and psycho-socially complex care with the
Directors of Behavioral Health and Medical Care Coordination.
Serves as a primary clinical contact with community partners to
partnerships and relationships with nursing schools and other
healthcare-related academic institutions to advance IF's mission
and improve client care and care coordination.
Qualifications:
Required:
RN with BSN/MSN with 5 years managed care experience with line
management responsibility including clinical operations
Demonstrated success in building high performing operating
teams
Proficiency in Google Suite (Mail, Docs, Sheets, Calendar).
Valid CA Driver's License and vehicle insurance.
