Job Description:
A company is looking for a Utah License Patient Monitoring LPN
or LVN.
Key Responsibilities:
Outreach to prospective patients and serve as the point of contact
for patients enrolled in the RPM365 platform
Help patients connect to the RPM 365 app and set up their
devices
Engage regularly with assigned patients, monitor their data, and
educate them through shared information
Required Qualifications:
Possesses a Nursing (LPN, LVN, RN) or Clinical Certification
(Social Worker, SW assistant, Dietician, etc.)
Knowledgeable about chronic health conditions and the operation of
devices used for monitoring
Proficient with technology, comfortable with computer platforms and
smart phone functionality
Experienced with client interaction and comfortable with telephone
outreach and engagement
Highly organized, efficient, and exhibits a strong sense of
ambition and self-discipline
