Registered Nurse (RN) | Case Manager Location: Mission Viejo, CA Agency: Revolution Healthcare Services Pay: $2,282 per week Shift Information: Days - 5 days x 8 hours Contract Duration: 13 Weeks Start Date: 2/2/2026 About the Position Job Opportunity – Medical Social Worker (MSW/LCSW) – Southern California Start Date: February 2026 Location: Mission Viejo, California Compensation: Details provided to qualified applicants Schedule: Sunday through Thursday, subject to facility needs. Every other weekend required. Caseload of 20-23 patients. 40-hour orientation provided. Highlights: Acute care discharge planning and case management in a dynamic hospital environment Work with adults and geriatric populations across multiple diagnoses Collaborate with interdisciplinary teams including RN case managers, physicians, and insurance coordinators Epic EMR system used for documentation and care coordination Initial assessment required within 24 hours; coordinate post-acute care needs and safe discharge planning Up to 7 days of requested time off may be accommodated Requirements: Active California state license (MSW or LCSW) or pending license that will be active by start date Minimum 1 full year of acute care experience required (non-behavioral health) Prior travel healthcare experience required; first-time travelers will not be considered 2 years of overall social work experience preferred Current BLS certification Proficiency with Epic EMR system required Experience with Trauma Level I or II facilities preferred Strong skills in discharge planning, care coordination, and case management Why It's a Great Opportunity: This role offers the chance to make a meaningful impact on patient outcomes by facilitating safe, timely discharges and coordinating comprehensive post-acute care services.
