Job Description:

The Direct Support Professional (DSP) at Care Partners At Home is a crucial member of our healthcare organization. We help individuals with developmental disabilities and dependent adults achieve their goals through self-empowerment and support of their daily needs and activities.

Our client is a visionary in the support of individuals with developmental disabilities and needs your help! We are searching for Direct Support Professionals to support participants in a beautiful new center in Santa Ana, CA. The center is full of amazing activities and therapies for our participants including a physical fitness facility, cooking stations, technology stations, Occupational & Physical therapies and countless other engaging activities.

As a Direct Support Professional, you would work directly with our participants, engaging with them during their time at the center, keeping them safe, supported and assisting them with achieving their goals. Additionally, our DSP Team assists with the following job duties:

Performing personal care tasks, including feeding, ambulation, and medical monitoring

Assists and trains persons supported in all independent living skills including: meal preparation, meal clean-up, bed making, laundry, dusting, vacuuming, sweeping, mopping, and other cleaning tasks

Assist with fostering positive relationships between individuals served and other participants at the center

Ensuring participant safety and maintaining a safe environment

Encouraging self-help activities

Maintains and promotes a friendly, comfortable, safe environment where people may progress socially, emotionally, and physically

Respects and promotes the rights of individuals

Qualifications:
Must be at least 18 years of age

Must have current First Aid and CPR Certificate

High School Diploma or equivalent; preferred

Experience working with developmentally disabled individuals; preferred

Read, write, comprehend, and communicate in English well enough to perform essential duties and responsibilities

Ability to maintain confidentiality

Communicate and work professionally and cooperatively with others

Successfully pass all post offer, pre-employment screening requirements, including criminal background check, drug screening, health screening, TB screening and provide proof of eligibility to work in the United States

Ability to run, push, pull, bend, squat, kneel, and lift up to fifty (50) pounds

Current Direct Support Professional I (DSP I) certification; preferred, not required

QUALIFICATIONS:
? Must have at least one year of employment experiences as a Basic Personal Care Attendant

? Has registered with the Home Care Aide Registry

? Affiliated Home Care Aids: Proof of completion of a minimum 5 hours of entry level training prior to client contact which must include but not limited to; two hours of orientation regarding his/her role as a caregiver and terms of employment; three hours of training focused on infection control, emergency procedures to also include a First Aide certificate.

? Ongoing proof of 5 hours annually to include but not limited to topics such as: client rights and safety, providing for and responding to client?s needs, reporting, detecting, preventing abuse and neglect, assisting clients with personal hygiene and other home care services.

? Certified Nursing Assistants: Proof of successful completion a minimum of 40 hours of training and/or demonstrated competency in and knowledge of:

o Responsibilities and limitations communication techniques

o Client rights confidentiality of PHI

o Respect for person and property

o Observation for client changes to report

o Standard infection control

o Home safety precautions

o Basic body functions and variances

o Procedures for clean, safe environment

o Emergency response

o Physical, emotional, and mental client care

o Bed, sponge, tub or shower bath

o Skin care, oral hygiene, toileting

o Basic nutrition, meal preparation

o Encouraging independent care

o Documentation techniques

o Supervisory support and duties

o Advance directives implications

? Ability to communicate with clients and to remain calm under stress

? Ability to read and write in English

? A state Certified Home Health Care Aide or state Certified Nursing Assistant or pass the agency requirements for a Basic Personal Care Attendant may perform these duties

? Ability to follow oral and written directions and complete work as assigned

? Ability to maintain confidentiality about client services and personal information

? Provide written proof of health status and ability to lift, bend, squat the level and amount determined by each job assigned

? Accurately document basic Personal Care Attendant services provided

? Present a neat appearance and positive work attitude

RESPONSIBILITIES:
1. To lead first by demonstrating a complete understanding of the Care Partners At Home mission: We will create deep meaningful relationships with our clients, their families, our associates, and partners resulting in the establishment of high energy environments focused on true person centered caring, while increasing the quality of their lives, and ours, within every interaction.

2. To engage their client with the purpose of understanding their unique needs so as to best represent, advocate and provide feedback to both the organization and family when appropriate.

3. Participates in and supports the agency?s performance improvement activities

4. Provides basic Personal Care Attendant services to clients as identified by the agency in the plan of services per agency policies, procedures, and Basic Personal Care Attendant skill level.

5. Prepares and maintains accurate and timely client notes, documenting interventions, client pain score, outcomes, and client response per agency policy.

6. Assists with personal care activities (ADLs) and IADLs allowing client independence to tolerance as indicated in the service plan including:
