Job Description:
The Peer Navigator Team Lead is part of our Working Wardrobes,
Veteran Behavioral Health Peer Support Services (VBHPSS) Contract
Team, co-located with the Orange County Health Care Agency (OC4Vets
Program) and County Veterans Services Office (VSO), designed to
work closely with the -OC4Vets Program and VSO to provide Veterans,
military-connected individuals, and their family members with a
support system to address barriers when accessing Behavioral Health
Services. The Peer Navigator Team Lead -will have direct report
management of the VBHPSS Contract Team, consisting of one (1)
Employment & Housing Specialist, and three -(3) Peer Navigators,
each carrying the -collective responsibility for conducting street
outreach, participant screenings, peer, employment, and housing
navigation sessions as stipulated in the VBHPSS Contract. Services
are short-term (3-6 months) and are intended to facilitate
confirmed linkage to appropriate services provided by OC4Vets or by
other Veteran service providers in the community to successfully
address bio-psycho-social barriers. -The -Peer Navigator Team Lead
will utilize shared knowledge of military culture and experience to
build rapport needed to engage Veterans and their family members.
This is to be achieved through -active listening skills and
motivational interviewing while applying an empathic approach
during all participant interactions. It is the responsibility of
the Peer Navigator Team Lead to provide over-the-phone and
face-to-face peer support to Veterans, military-connected
individuals and their families while navigating the service system
through the provision of accurate information, assistance and
referral, proactive check-in, follow-up assistance, program
enrollment and advocacy with the ultimate goal of helping our
participants receive the services they need. -
DUTIES AND RESPONSIBILITIES -Reporting to the Director, Client
Services, this role is responsible for but not limited to: - -
- Achieve all VBHPSS Contract Productivity/Units of Service and
Performance Outcomes. -
- Oversee and Manage Individual and Team adherence to guidance
detailed in the VBHPSS Contract SOP. Oversee and Manage Individual
and Team adherence to guidance detailed in VBHPSS Contract Flexible
Funds Policy. - -
- Oversee and Manage Individual and Team adherence to guidance
detailed in VBHPSS Contract Vehicle Policy. -
- Oversee and Manage Individual and Team adherence to proper
completion of all VBHPSS Contract Administrative Forms, Data Entry
and Reporting. - -
- Track, Manage and Report all VBHPSS Contract Flexible Funds
(Supportive Services rendered to Participants).
- Oversee and Manage planning, scheduling, and assignment of VBH
Peer Team Community Outreach and Event Engagement. -
- Oversee and Manage VBH Peer Navigation Team Participant
Caseload Assignments through completion of goals planned in
Participants Individualized Employment/Support Plan. Assist VBH
Peer when/where need presents to achieve positive outcomes. -
- Track, Manage and Report Participant Intake/Enrollment via
Participant Log. -
- Track, Manage and Report VBHPSS Contract Units of Service for
duration of Contract Year. -
- Oversee and Manage VBH Peer Weekly Narrative Reports and Submit
Monthly VBH Peer Team Narrative Report.
- Distributing and collaborating for peer advocacy and self-care.

- Working on retention and assign peers to other projects. -
- Responding to and conduct program screenings, over phone or
in-person. -
- Conducting research to locate and maintain resources for
Veterans and their families. -
- Assisting veterans with navigating systems of care as requested
by the on-site supervisor or designee.
- Providing information or refer Veterans to public or private
agencies or community services for assistance. -
- Working with agency staff and/or family members to decide which
clients can work with the navigator. -
- Conducting an initial assessment with each participating
consumer to assess his/her health and wellness status and
experience with getting health care. -
- Collaborating with the consumer and, in some cases his or her
family (and/or care team) to set healthcare and wellness goals.

- Navigating the medical system with the consumer including such
things as helping the consumer make medical appointments, going
with the consumer to appointments, helping the consumer communicate
with medical personnel and/or coaching the consumer about ways to
talk with medical personnel, helping with any follow-up necessary
such as lab tests, getting prescriptions filled or taking paperwork
to the consumer's mental health provider.
- Coordinating the consumer's navigation and goals with
appropriate agency staff. -
- Communicating with clients on a regular basis to keep them
engaged and to assess progress. -
- Tracking the progress of clients including conducting
follow-along assessments. -

helping clients overcome personal barriers. -

different clients on their goals, juggling different priorities
while at the same time flowing with the many changes we incur as we
constantly adjust to meet the needs of our veterans. - -
- An eye to the future--You'll take a proactive approach to
monitoring the needs of Veterans and current -market trends. -
REQUIREMENTS -Education and Experience: -
- Military Veteran, or spouse/partner of active-duty military or
Veteran, status is required. -
- Must have specialized training in working with active duty,
Veterans, and military households. -
- One-year Information and Referral experience or extensive
knowledge of health and social services in Orange County required.
-Skills Set: -
- Bilingual English/Spanish highly desirable. -
- Demonstrated excellent verbal and written communication skills
required. -
- Demonstrated proficiency in web-based systems, chat, and
Microsoft Suite including Teams, Access, Outlook, Word, Excel, and
PowerPoint. A Skills test may be required. - -PHYSICAL REQUIREMENTS
OF THE POSITION -
- Able to remain in a stationary position (both sitting and
standing) 50% of the time or more. -
- Able to move about the office to access file cabinets, greet
clients and volunteers, etc. -
- Able to move, traverse - move to access documents, files,
supplies, etc. -
- Frequently communicate with donors and volunteers who have
inquiries about the organization's mission and services. - -
-BENEFITS: -This full-time position is eligible for medical,
dental, and vision coverage for up to 80% coverage on a base plan
for the employee as well as generous paid holiday, sick, and
