Job Description:
A company is looking for a Coordinator - Social Services.
Key Responsibilities
Obtain and share information on benefits and community resources to
support patients after discharge
Assist in sending Medicaid applications and other necessary
documentation for patient benefits
Collaborate with nurse coaches to ensure patients have access to
community resources for recovery
Required Qualifications
Degree in social work or a related field
Deep knowledge of community amenities and resources
Understanding of medical terminology and planning
Experience in recognizing Health Plan benefits and community
resource needs
5+ years of experience in community resource management or Health
Plan benefit management
